Home Health Aide
Job Description
Provides hands‐on personal care and assistance with activities of daily living, such as bathing, dressing, grooming, and toileting, in accordance with the patient's individualized plan of care
Delivers care under the supervision of a Registered Nurse, following New York State Department of Health guidelines and all agency policies and procedures
Encourages and supports patients in performing tasks independently to promote self‐care and maintain dignity
Assists with light housekeeping tasks to ensure a clean, safe, and organized home environment that supports the patient's well‐being
Prepares and serves meals using basic nutrition principles and modifies diet plans as directed by the Registered Nurse to meet the patient's health needs
Provides care using infection prevention practices and reinforces infection control education given by nursing staff
Aids patients with prescribed rehabilitative exercises under the guidance of a Rehabilitation Therapist to support recovery and mobility
Observes the patient's physical, emotional, and behavioral condition and accurately records daily care activities and any significant changes
Communicates changes in the patient's condition immediately to the Registered Nurse or, if unavailable, to the Home Health Aide Supervisor
Participates as a valued member of the care team, offering feedback and suggestions to improve the quality and continuity of patient care
Completes all documentation in a timely, thorough, and accurate manner according to agency standards and regulatory requirements
Provides compassionate, respectful, and culturally sensitive care while maintaining patient privacy, dignity, and comfort in the home setting
Minimum Required:
High School Diploma or GED and successful completion of a Home Health Aide program within 12 months of hire, OR
Current Home Health Aide Certification (no diploma/GED required if you're currently certified)
Work Environment
- Work is predominately in patient homes. Some work is performed in offices that require working in close quarters with other employees. The offices are air conditioned and heated. The job requires the following physical demands: Ability to work in a non‐sterile, unpredictable environment (furniture, other residents, icy walkways), including exposure to animals. Ability to stand and walk without restrictions. May require standing for 30 minutes or longer at one time. Ability to walk up flights of stairs carrying aide bag and supplies. Ability to bend, stoop, squat and twist. Ability to lift, push, pull patients a minimum of 50 lbs without restriction. Ability to independently transfer or assist patients to move, stand or walk in a safe manner. Travel between locations as required.
