Hospice Admissions Nurse
Job Description
This is a community-based role and will require regular travel through the borough of choice.
The compensation structure comprises both base pay and bonus pay.
We service: Nassau county, Queens, Brooklyn, Manhattan, and the Bronx. Please make sure to choose the area you are applying for in the screening questions.
The Admissions Registered Nurse (ARN) is responsible for evaluating referrals and coordinating patient/family needs for hospice admission. Participates in the development of the initial Interdisciplinary Plan of Care. Provides nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the Interdisciplinary Plan of Care. Supports the Provider Relations and Quality Improvement activities of the organization. Works under general supervision.
QUALIFICATIONS:
1. Registered nurse currently licensed in the New York state required.
2. Baccalaureate Degree in nursing from an approved program; or
a. Baccalaureate Degree in nursing from a non-approved program supplemented by content which can be equated to the approved program; or
b. Registered professional nurse with two yearsβ experience in either home health care or Hospice nursing.
3. Minimum one year Med/Surgical or other professional nursing experience relevant to assigned case management preferred.
5. Clinical competence in nursing and the ability to multitask in a fast-paced environment, listen attentively/evaluate calls/use judgment in a time sensitive environment required.
6. Effective oral/written communication, customer service and interpersonal skills required.
7. Knowledge of Computer operation, computer generated clinical records experience preferred.
8. Demonstrated commitment to Hospice philosophy of care.
9. Must have a valid driverβs license, motor vehicle, and minimum state required liability automobile insurance.
10. Bilingual skills, preferred
Job Type: Full-time
Pay: $90,000.00 - $125,000.00 per year
