Hospice RN Case Manager

Allstar Hospice is looking for a compassionate and experienced Hospice Nurse (RN) to join our case management team in the Southwest Houston area. This position can be Full Time or Part Time The primary functions of our RN Case Manager are to provide nursing care for hospice patients either in their place of residence, a skilled nursing facility or an assisted living facility. The duties of the RN Case Manager are to assume responsibilities of the coordination of care between the patient, healthcare providers, team members, as well as their families. Required: Current RN License Valid Texas Drivers License and Auto Insurance At least 2 years of hospice and/or home health nurse A sympathetic and compassionate attitude Preferred: Bilingual in both English and Spanish Job Types: Full-time, Part-time Pay: Up to $85,000.00 per year Expected hours: No less than 40 per week Benefits: • Dental insurance • Free parking • Health insurance • Paid sick time • Paid time off • Vision insurance Experience: • Hospice care: 2 years (Required) • Case management: 2 years (Required) License/Certification: • RN (Required) Work Location: In person
