• Northwest suburbs: Outpatient clinic located in Woodstock, IL with hospital privileges at Northwestern Medicine Woodstock, McHenry, and Huntley Hospitals
• West suburbs: Outpatient clinic located in South Elgin, IL with hospital privileges at NM Delnor hospital.
