Kidney Hero Navigator
A Kidney Hero Navigator is responsible for the execution of Kidney Navigator program for patients requiring dialysis at acute or hospital discharge. This will be done through patient education, referrals, patient engagement and transition to longitudinal management. This position enhances the patient experience with education and shared decision making.

Essential Functions
- Meets with patients requiring outpatient dialysis placement or for Chronic Kidney Disease (CKD) education as defined by program.
- Educates Re: Progression of disease for Chronic Kidney Disease (CKD), Transplant, Dialysis, Modality options, Access options, insurance which includes an assessment of patients’ insurance options.
- Assists the patient to navigate options for outpatient dialysis modality of their choosing. Work with care teams to resolve any barriers to placement.
- Demonstrates empathy for individuals, based on a thorough understanding of patients’ individual needs, enthusiastically present Strive’s care model.
- Nimbly address inquiries and objections. Â
- Provides a hand-off to Strive’s Enrolment Associates, sharing what you’ve learned about the patient’s unique situation and ongoing needs through our care management platform to enable an excellent patient experience.
- Provides quality hand off to departments post discharge program in the event that the patient chooses to not enroll in Strive’s care program
- Ensures patient satisfaction
- Collaborates with hospital case managers, social workers, and physicians to prepare patients for dialysis.
- Arranges kidney dialysis therapy for patients by referring to local providers within an appropriate proximity as determined by patient preference.
- Supplies HIPAA appropriate renal information and medical documentation to care team.
- Documents referrals in accordance with the program policy and procedure.
- Works closely with kidney care professionals to enhance current relationships or develop new relationships.
- Collaborates with care teams to employ best practices for transitions of patients across care settings.
- Attends team meetings with care teams in assigned region.
- Works in collaboration with all members of the care team and acts as a patient advocate.
- Subject matter expert in management of Chronic Kidney Disease including transplant, dialysis modality, patient engagement and understanding of transitions of care for chronic kidney disease.
- Escalates barriers to additional subject matter experts.
- Prepares the patient for longitudinal management of disease.
- Efficiently and safely manage all systems that house PHI
- Follows all policies and procedures related to safe handling of PHI.
- Meet in person with internal and/or external stakeholders to facilitate team and business priorities/opportunities.Â
- Serve patients in person and in multiple care settings (e.g., patient home, clinics) in addition to virtual visits.  Â
Minimum Qualifications
- High School diploma
- 1 year experience in discharge planning/hospital acute care/dialysis
- Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 msÂ
Preferred Qualifications
- Licensed LPN/Social Worker/Dialysis Technician
- 1 years in dialysis setting
- Healthcare experience
- Insurance knowledge
- Case management
Physical/Environmental Demands
- Ability to reach overhead and side to side
- Lifting overheard and carrying of 10lbs routinely, 20lbs sporadically
- Ability to sit, stand and kneel as needed to perform physical assessment
- Ability to grasp and hold medical instruments as needed to perform physical assessment
- Ability to navigate stairs and a variety of patients dwelling conditions and configuration
Hourly Range: $21.05-$24.03
