Wage: $22.80 per hour
Job Purpose:
The Location Coordinator is equally responsible for overseeing the location by ensuring the health and safety of the persons served, maintaining the physical site, and supervising employees. The location coordinator provides assistance, support, and encouragement to meet the needs of people served by HMC. Services are provided in the individuals' homes and in the community. Minimally, you will be scheduled to work generally Monday – Friday with the expectation that the direct care shifts will be filled with admin hours falling around these shifts. Some weekends as needed.
Location: Available in Mankato and North Mankato
Duties and Responsibilities:
- Provides supervision and assistance to individuals and team members in a genuine, person-centered, and respectful manner while maintaining professional boundaries and actively participates in individual meetings as requested.
- Assists individuals with activities of daily living.
- Encourage participation in leisure activities at home and in the community and promote a healthy lifestyle.
- Support and/or provide individuals with medical needs, including medication administration and medical appointments.
- Monitors and assists with the personal finances of persons served.
- Maintains cleanliness of the location and ensures safe working and living environments.
- Responsible for oversight of programmatic finances, location functions, equipment upkeep, and vehicle maintenance.
- Completes required documentation and communicates information for programming, shift report, and maintenance.
- Provides direct supervision to employees assigned to the location including scheduling, coordination of orientation, staff meetings, and performance management.
- Demonstrates exemplary supervision skills, problem solving, and responsible work habits.
- Facilitates a supportive, safe, and team-oriented work environment.
- Displays exceptional documentation skills, knowledge of systems, routines, and programming.
- Attends all staff meetings and completes all required trainings and in-services.
- Fulfills duties and roles of On-Call when scheduled.
- Assists with the planning, development, and implementation of person-centered plans.
- Ensures the location schedule is adequately filled and all shifts are covered.
- Follows all health and safety policy and procedures
- Maintains HIPAA Compliance in all communications, whether written or verbal, both external and internal to HMC.
- Other duties as assigned.
Qualifications:
- Must be at least 18 years of age and have a high school diploma or general education degree.
- Preferred minimum of 6 months supervisory experience or have equivalent education in a related field.
- Preferred experience as a CNA, PCA, Caregiver, DSP, or healthcare related professional.
- Must have a valid Driver’s License and pass a driving record check conducted by our insurance carrier, unless otherwise noted.
- Excellent verbal and written communication skills - with a strong command of the English language - including the ability to effectively communicate with individuals, support teams, and co-workers.
- Must be self-motivated and exhibit strong interpersonal communication skills, able to problem solve and respond effectively to emergency situations.
- Must demonstrate ability to facilitate conflict resolution among staff and persons served, with a general knowledge of behavioral concepts.
- Must display a positive attitude towards the job.
- Must exhibit strong advocacy skills.
- Must pass a MN Department of Human Services Background Study.
Physical RequirementsÂ
- Ability to perform the essential job functions safely and successfully, consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
- Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
- Must have the ability to regularly kneel, crouch; occasionally required to sit, climb and balance.
- Must regularly lift and/or move up to 10 pounds; occasionally lift or move 50 pounds.
