Manager of THRIVE Operations
Job Description
POSITION SUMMARY:
Reporting to the Director of Innovation, Health Equity Accelerator, this position has the primary responsibility of supporting and coordinating the operations of THRIVE, BMC's social determinants of health program. This position will help develop, implement, and monitor work plans that enable THRIVE screening, referrals and special projects.

ESSENTIAL DUTIES/RESPONSIBILITIES:
THRIVE Screening
- Support clinical departments with increasing THRIVE screening rates to meet organizational goals by
- Forming department-level plans to increase screening rates
- Implementing regular review of department-level metrics to monitor performance
- Following up and strategizing with departments who are following below screening requirements
- Lead onboarding of new departments with THRIVE screening and referral tools
- Support capacity building efforts in social determinants of health (SDOH) screening among accountable care organization (ACO) partners in coordination with population health team
THRIVE Referrals
- Coordinate with third-party vendor to update and provide training on referral guides and updates to THRIVE resource directory
- Coordinate with third-party vendor to complete and maintain user scoping matrix for all current platform users to understand workflows, platform gaps and training opportunities
- Proactively integrate offline organizational lists managed by individual navigators or care teams into the THRIVE directory platform
- Work with care teams to update referral guides and reference materials for THRIVE
- Implement and train care teams on new or expanded THRIVE directory functionality
THRIVE Special Projects
- Proactively recommend and develop processes and methodologies to improve THRIVE processes
- Provide project management support for special projects and initiatives identified by THRIVE leadership
- Integrate feedback and insights from patient advisory group into THRIVE work plans
Other Responsibilities
- Support agenda development and actively participate in all THRIVE meetings
- Assist with follow-up items from the discussion or decisions made from THRIVE meetings
- Performs other duties as required.

JOB REQUIREMENTS
EDUCATION AND EXPERIENCE:
- At least five years of relevant work experience in large and complex health care organization
- At least two years of experience supervising front-line clinical staff
- Bachelor's degree preferred but not required
KNOWLEDGE AND SKILLS:
- Superior organizational and interpersonal skills
- Excellent written and verbal communication skills
- Strong project management skills, project management training preferred
- Strong collaboration and problem-solving skills
- Strong meeting facilitation and presentation skills
- Demonstrated effectiveness as a team member
- Proficiency in computer and internet skills, experience with Epic preferred
