WelbeHealth PACE (All-Inclusive Care for the Elderly) program provides seniors with the opportunity to continue living in their homes and in their communities. Our innovative and comprehensive range of medical services to participants is what ignites our passion to treat the whole person and not the symptoms! We employ a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care, which is key to WelbeHealth values, team culture, and mission.

The Medication Coordinator works with the clinical team, supporting the clinic nurse to ensure participants receive their prescriptions in a timely manner. The Medication Coordinator focuses on entering and ensuring key participant information is updated and accurate in all systems, and providing instructions to participants living in private homes or Long-Term Care on the packaging, delivery, and refill processes of all medications.

Essential Job Duties:

• Ensure prescriptions are filled timely to avoid gap in prescription refills as new participants enroll in WelbeHealth
• Ensure participant information primarily enrollment status and delivery address are kept up to date in EireneRx and other systems as required
• Address medication request (new, change, d/c) issues/questions and escalate as needed
• Arrange for delivery of participant medications from local pharmacy or from the Center
• Coordinate medication orders and changes for participants in assisted living facilities and skilled nursing facilities as needed
• Coordinate arrangements for diabetic supplies and equipment for new participants

Job Requirements:

• High School Diploma or equivalency
• Experience as Medication Medical Assistant, Med Tech, or Pharm Tech preferred
• Two (2) years of experience in a clinical setting with a frail or elderly population, preferred
