Role Description:
The goal of this role is to support our Illinois care teams by discovering and ingesting a comprehensive medical history on all new patients into Oak Street's patient records to facilitate efficient and effective medical care for our patients. This role will do this by reviewing external electronic medical records (EMRs) as well as following up via phone or fax with additional specialist and primary care offices. This role will work closely with regional Population Health and clinical leadership to correctly index and file all medical records according to clinical relevance and importance to help care teams review patient information more quickly.

Role Description:

Core Responsibilities:
- Manage and maintain electronic medical record (EMR) access to all relevant health systems in the IL market
- Access all EMRs in IL for every new patient to mine for medical and social histories for those patients
- Upload all discovered external information into Oak Street's EMR, filing to the appropriate document types to facilitate easier clinical review
- Initiate and follow up on additional phone and fax medical records requests where direct EMR access is not possible
- Partnership with Population Health Directors, Clinical Documentation Specialists, and regional clinical leadership on clinical significance indexing and other ways to improve the ease of new patient chart reviews
- Other duties as assigned
