Short Description

The Advanced Practice Provider (APP) - Nurse Practitioner or Physician Assistant - Home Visiting

The Advanced Practice Provider (APP) provides in-home post hospital follow up care to adult patients recently discharged from the hospital or skilled nursing facility.  An APP home visit affords high risk patients access to proactive, comprehensive, compassionate, and collaborative care to improve their health and well-being, while keeping them home.

Responsibilities include but are not limited to:

  • Perform in-home comprehensive physical and social patient assessments.
  • Develop a comprehensive treatment plan for each patient.
  • Medication Reconciliation and management.
  • Order and interpret diagnostic tests.
  • Communicate test results to patients and their caregivers.
  • Provide patient and care giver education.
  • Collaborate with the interdisciplinary team.
  • Partner with population health for continued care coordination.
  • Document in patient’s electronic health record.

 

APP Home visit service area includes Burlington, Camden and Gloucester counties.

Experience Required

Minimum of two years of experience working with the adult or senior population. Current knowledge of chronic care/geriatric care and best practices.


Education Requirements

Master’s Degree in Nursing or Physician Assistant degree is required.


License/Certification Requirements

Certification: American Heart Association BLS certification is required

License: NJ-APN & RN or NJ PA License is required

 

Valid State Driver’s License with a clean DMV record.

Copy of recent Vehicle Insurance required.


Special Requirements

Scheduled Days / Hours: Monday-Friday: 8:00am-4:30pm

Sign On Bonus $10K with a 2 year commitment
