All FORM patients work closely with their care team, which includes board-certified physicians, advanced practice professionals, and Registered Dietitians. Our proprietary mobile app lets patients engage in regular video visits, text messaging, photo journaling, digital data transmission, and customized educational materials. About the Role:

We seeking an Advanced Practice Professional who is passionate about and experienced in obesity medicine to provide longitudinal Telehealth care for patients with obesity and collaborate with physicians and registered dietitians to provide exceptional team-based care. This is a full-time opportunity; work is remote, and schedules can be very flexible.

We seek applicants who:

• Have an NP or PA degree, with experience practicing Obesity Medicine, including effective evidence-based lifestyle counseling and pharmacotherapy
• Are licensed to practice and prescribe legally in Massachusetts, New York, New Jersey, Washington, Illinois, California, Virginia, Kansas and Texas, and/or other states, and will prioritize candidates with multiple licenses
• Have at least 2 years of Obesity Medicine experience
• Have excellent interpersonal skills for patient and team interactions
• Are committed to the highest quality of medical care and an evidence-based approach to obesity management 
• Have prior experience conducting telemedicine visits
