On Call Hospice RN

Oasis Hospice is seeking aOn Call Hospice Nurseto care for our families in Cook, DuPage and Will Counties. Requirements include: - Current RN Licensure in the State of Illinois. - Excellent observation, verbal and written communication skills. - Must be a licensed driver with an automobile that is insured in accordance with state of Illinois requirements and is in good working order. - Willingness to travel. Work Hours and Frequency The On Call RN must be available to be on-call during the week day, Monday - Thursday, 5pm - 8.30am, Also Friday 5pm - Monday 8.30am. One week On and One week off. Job Responsibilities: The On Call Registered Nurse is responsible for providing skilled nursing, intervention, and supportive care to patients in the home setting for weekend and holiday on-call duty. - Answer all after hours phone calls received on behalf of the Agency, and follow up according to Agency policies and procedures. - Provide skilled nursing, intervention, and supportive care to patients in the home setting. - Provides professional nursing care by utilizing all elements of nursing process. - Administers medications and treatments as prescribed by the physician in the physician’s plan of care. - Provides health care instructions to the patient as appropriate per assessment and plan. - Educate primary caregivers to provide care for patients as indicated. - Maintain regular communication with agency staff and the patient’s referring physician and/or case manager concerning patient/family care needs and response to interventions. - Complete documentation within agency guidelines and to meet regulatory requirements. Submit completed documentation by midnight on the day on-call services are provided. Compensation - The On Call Registered Nurse will be paid a salary. - Attendance at scheduled staff meetings, orientation or training sessions, or other assigned duties will be paid at the agreed upon hourly rate. - Mileage accrued through travel required for the job will be reimbursed at the standard agency rate.
