Who we’re looking for:

The Outreach and Engagement Coordinator will work with the Director of Business Development and facility management to develop and implement a community outreach plan to spread awareness of Forge Health services for his or her assigned territory in the New York area. This position will be responsible for outreach and business development for KPC as well as Forge New York. Specifically, this territory includes Long Island, New York City (Queens), and Westchester Counties.

The Outreach and Engagement Coordinator will specifically be responsible for developing strategic relationships and partnerships. This individual must have a commitment to excellence and always be focused on customer service and relationship development. The ideal candidate will have excellent interpersonal skills and works as a team-player to build rapport with various community groups and referral organizations. Additionally, this position requires frequent travel as well as the ability to attend networking events on evenings and weekends as needed in order to promote the Forge Health brand.

What you’ll be doing:

  • Identify and develop relationships with new community contacts and organizations in effort to grow Forge Health’s referral base in the Long Island/NYC/Westchester area, including but not limited to hospital systems, primary care doctors, detox centers, psychiatric emergency services, and other behavioral health and community providers.
  • Attend community events, county meetings, and coalitions related to the behavioral health field to promote Forge Health services and specialty programs and maintain a strong presence at all events.
  • Develops and maintains relationships with community members, currently enrolled families, and prospective families who can effectively advocate on behalf of Forge Health.
  • Develop events, office tours, and educational forums that educate patients, families, community and other referral sources on Forge’s services.
  • Create outreach initiatives that will regularly communicate facility mission with referral sources through phone, email, e-blast, written notes, events, networking, and other professional activities.
  • Support Access Center of Excellence (ACE) and Care Coordination teams to maintain a client preferred provider guide to ensure resources and referrals are up to date and easily accessible Forge Health clients.
  • Respond to inquiries from community members and other interested parties
  • Ensure relevant documentation is completed to maintain business development referral records.
  • Accountable for satisfying all job specific obligations and complying with all organization policies and procedures.

  • Minimum Bachelor's Degree
  • Minimum of two (2) years of job-related experience
  • Working knowledge of health care environments, clinical terminology, and health information systems
  • Must have a valid state license and access to a vehicle
