Monogram Kidney Care supports patients suffering from CKD and ESRD by forming deep-rooted relationships and preparing them both emotionally and physically for the challenges of managing kidney disease. We use in-home visits to build relationships that drive positive behavior change. Monogram is seeking a full-time, board-certified, hospice / palliative care MD, licensed in Phoenix AZ. Treatment and oversight will occur through both telehealth and in person.

The physician will connect with CKD and ESRD patients appropriate for conservative care and engage with patients who have multiple complex co-morbidities. Monogram Health allows physicians to practice medicine in a forward-looking, evidence-based environment along with others who embrace the trends toward telehealth and value-based care. Monogram physicians promote palliative care where clinically appropriate and focus on education, autonomy, and quality of life.

The Palliative Care physician will collaborate with the POD Medical Director to identify CKD members, own the outcomes for the patient panel, treat both virtually and in-home, determine appropriateness of care, conduct meaningful end-of-life discussions, reduce preventable episodes of care, and develop key relationships with local providers. Top candidates must have 5+ years of experience, a current AZ state license, board certification in IM, Hospice & Palliative Care, and demonstrated experience applying evidence-based clinical criteria.
