Palliative Care Leader in Phoenix AZ
Job Description
The physician will connect with CKD and ESRD patients appropriate for conservative care and engage with patients who have multiple complex co-morbidities. Monogram physicians promote palliative care where clinically appropriate and focus on education, autonomy, and quality of life.
The Palliative Care physician will collaborate with the POD Medical Director to identify CKD members, own the outcomes for the patient panel, treat both virtually and in-home, determine appropriateness of care, conduct meaningful end-of-life discussions, reduce preventable episodes of care, and develop key relationships with local providers. Top candidates must have 5+ years of experience, a current AZ state license, board certification in IM, Hospice & Palliative Care, and demonstrated experience applying evidence-based clinical criteria.
