Medication Therapy Management Pharmacist Position Summary 

The Medication Therapy Management (MTM) pharmacist is responsible for performing comprehensive and targeted MTM for identified patients of TidalHealth.  Works together with pharmacy leadership to implement, coordinate, and communicate the functions established to deliver effective, and efficient patient focused MTM services. The MTM pharmacist assures the effective integration of operational and clinical resources throughout the MTM process in order that the pharmacy achieves the highest level of safe and effective care of patients. Works with pharmacy staff, medical staff, patients, and nursing staff as necessary to assure comprehensive MTM services are delivered.

Additionally, the Pharmacist – Medication Therapy Management coordinates, on a day-to day basis, the provision of high quality, comprehensive pharmaceutical care to the patients of TidalHealth. This includes: (1) defined clinical/cognitive activities to provide patient-centered services to optimize medication therapy and improve patient outcomes: (2) complete accurate and thorough medication histories and provide verbal educational activities with patients to help them understand how to use their medications properly: (3) provide education to pharmacist colleagues and other health care professionals (e.g. physicians, nurses, etc.) to maintain and advance clinical competency within drug therapy.

Medication Therapy Management Pharmacist Position Requirements  

• BS in Pharmacy or PharmD from an accredited School of Pharmacy.
• Current pharmacist licensure in the primary state in which they will work and licensure in both Maryland and Delaware within 6 months of hire.
• PGY1 residency preferred

Medication Therapy Management Pharmacist Benefits
