Pharmacist - Member Outreach HealthCare Support is actively seekingRemote Pharmacist - Member Outreachto fill multiple openings with our prestigious healthcare client. Must have reside and have active California Pharmacist License. Daily Responsibilities for Pharmacist - Member Outreach: • Member outreach for medication adherence and meeting quality goals, day to day duties include: • Member facing, doing telephonic outreach • Transitional Care Services- clinical rounds with Case Management, receive clinical referrals. • MRP- post-hospital discharge medication reconciliation. • Assist with clinical outreach for cardiac conditions such as hyperlipidemia, hypertension, etc • M-F, 8-5 PST or 9-6 PST must be comfortable working either of these schedules. must be flexible to work one or two weekend days per month, depending on need. • 6 month contractRequired Qualifications for Pharmacist - Member Outreach: • Pharm D required • Clear and active Pharmacist license in CA • 2+ years of Pharmacist experience working for another Health Insurance Plan or hospital setting • MUST have medication reconciliation experience Preferred Qualifications for Pharmacist - Member Outreach: • Post hospital discharge, Outreach experience Benefits for Pharmacist - Member Outreach: • Immediate enrollment in Health Insurance • Dental Insurance • Life Insurance • Employee Assistance Program (EAP) • Access to Investment Accounts Pay: $60.00 to $65.00 an hour Interested in being considered?
