Strive Care Partners Overview
Strive Care Partners (SCP) is a dedicated business unit within Strive Health that partners with nephrologists to share and succeed in global risk contracts with payors. SCP will benefit patients with chronic kidney disease (CKD) and end stage kidney disease (ESKD) through a comprehensive, patient-centered approach that enhances lives and local communities. SCP develops local market Managed Service Organizations (MSOs) that aggregate networks of nephrologists and provide them with the tools necessary to be successful in managing risk on their patients. Specifically, SCP delivers primary care services (StriveCare365), specialized clinical programs, data integration and analytics, care teams, and administrative support services. SCP contracts with local market payors on the nephrologists’ behalf to take global risk on their CKD and ESRD patients and provides nephrologists with meaningful short-term and long-term financial incentives, including participation in shared savings and equity-based programs.

Practice Manager
The Practice Manager (PM) is responsible for coordinating all operational aspects across multiple clinic locations in their region. In partnership with market operations and clinical leadership, the PM will support the people and culture, as well as operational needs across our StriveCare365 Primary Care Clinics in order to ensure optimal practice performance with the overall goal of creating an exceptional patient experience.

Essential Functions
- Ensures that clinic operations run smoothly and timely on a daily basis
- Responsible for managing staff schedules across locations to ensure each practice has appropriate coverage
- Coordinates coverage if staff are out of the office – a successful PM is willing to step in to provide coverage on the front lines as appropriate (ex. opening or closing a clinic, scheduling patients, etc.)
- Manages and communicates clinical on-call schedule
- Management of contracted relationships required to operate the practices (i.e. stericycle, shred-it, McKesson etc.)
- Responsible for ensuring their team maintains an appropriate and adequate clinical supply and inventory in the office, while simultaneously remaining fiscally responsible
- Fosters a culture centered on the highest degree of patient experience and safety
- Responsible for team Quality performance including STARS and HEDIS gap closure
- Identifies and coordinates both routine and non-routine facility and equipment maintenance
- Oversees staff competency completion and performs documentation auditing
- Staff management including all day-to-day supervision of assigned direct reports, hiring, performance management, staff development, and performance reviews
- Facilitates team huddles
- Contributes to process improvement and standardization
- Supporting and contributing to on-the-ground marketing initiatives
- Participates in patient and provider outreach within their community
Minimum Qualifications
- Bachelor’s Degree in Nursing, Healthcare, Business, or related fieldÂ
- 3+ years of progressive experience in practice management; experience coordinating operations across multiple sites of practice preferred
- 3+ years of management/leadership experience within the healthcare or related fieldÂ
- Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 ms
Preferred Qualifications
- Master’s Degree in Nursing, Healthcare, Business or related field.
- Ability to teach, strong organizational skills, excellent communication, customer service, relationship development, results orientation, team building and decision-making.
- Understanding of Value Based CareÂ
- Change management experience
- Demonstrated ability, or potential, to excel in smaller entrepreneurial organizations.
- Excels at developing strong patient/family relationships that fosters engagement and best outcomes
Annual Salary Range: $83,700.00-$101,500.00
