- Our client, is seeking a Program Coordinator - Quality Developmental Disabilities Professional (QDDP) within their Day Support Program with one to six months experience working with persons with disabilities in a private home, group home, day support, or other facilities.

Job Description
 Duties and Responsibilities:
- Conduct comprehensive assessments to identify individuals' needs and strengths.
- Represent the agency at Individualized Service Plan (ISP) meetings and collaborate with stakeholders to develop Person-Centered ISP plans.
- Facilitate intake screening, admissions, and discharge planning processes.
- Coordinate services with support coordinators and ensure all staff are trained on ISP outcomes.
- Prepare reports including Person-Centered Reviews (quarterlies) and update ISP plans as needed.
- Market agency services to Community Services Boards (CSBs) and the general public.
 Knowledge, Skills, and Abilities:
- Ability to collaborate effectively with executive leadership to ensure program success.
- Proficiency in regulations governing developmental disability services, such as DBHDS, Office of Human Rights, Medicaid, OSHA, and Community Services Boards.
- Skill in conducting timely quarterly and annual reviews to assess individual progress and program effectiveness.
- Expertise in reviewing progress notes, training schedules, and other documentation to ensure alignment with Individualized Service Plans (ISP) and Person-Centered Plans (PCP).
- Capacity to conduct monthly individual record reviews to maintain accurate and up-to-date documentation.
- Experience in developing and implementing Individual Service Plans (ISPs).
- Strong organizational skills with the ability to manage time effectively.
- Broad understanding of various disabilities and person-centered planning principles.
- Excellent communication skills, both verbal and written.
- Positive attitude and passion for working with diverse populations.
- Ability to establish and maintain relationships with individuals, families, and community stakeholders.
- Proficient in problem-solving and solution formulation.
- Previous experience working with individuals with disabilities and conducting comprehensive assessments.
In this hybrid role, you'll be instrumental in creating and implementing Individual Service Plans (ISP) that empower individuals to achieve their fullest potential.
Qualifications
Requirements:
- Bachelor's degree in a human services field and at least two years of full-time experience working directly with individuals with developmental disabilities.
- Must have person-centered thinking skills.
- Certification in CPR, First Aid, and Behavior Intervention required.
- Successful completion of Criminal History and Central Registry background investigations.
 Physical Demands & Working Conditions:
- Working involves traveling and visitation to individual homes of varying sanitary and health (emotional and physical) conditions. Incumbent is also required to regularly transport individuals in the community.
- Regularly required to use hands to type, finger, handle, feel or reach with hands and arms.
- Have the ability to hear whispers with or without the aid of a hearing aide.
- Frequently required to stand, walk and sit; occasionally required to limb, balance, kneel or stoop, crouch or crawl.
- Frequently lift or move 10 pounds and occasionally lift 30 pounds. Intermittent physical activity including lifting and supporting clients may be required.
- May be exposed to viruses, diseases or infections from participants or specimens in working environment.
- Will be required to work at any facility, responsible for own transportation.
- Must maintain good driving record.
- Must maintain a registered vehicle.
- May experience traumatic situations including psychiatric and deceased patients.
- May be exposed to hostile and/or aggressive individual behavior, presenting risk of bodily harm.
- This is a hybrid position.Â
