This is a Full Time salaried position
ESSENTIAL FUNCTIONS:
β’ Oversee/Manage Discharge Plannerβ’ Collaborate closely with hospital discharge planners, social workers, case managers, physicians and
management, as well as provide education staff to generate referral sources.β’ Develop strong relationships with key referral sources.β’ Identify opportunities for partnering and growth.β’ Manage leads from referral sources and ensure efficient admissions process for clients.β’ Educate patients, families, community and other referral sources on programs, services and amenities
provided within facility.β’ Travel frequently within community, scheduling meetings and developing contacts/referral sources.β’ Document referral sources within appropriate contact management system.β’ Develop events and educational forums that demonstrate programs and provide an opportunity to
identify potential referral sources.β’ Maintain existing referral relationships.β’ Represent facility in professional forums and communicate mission in all outreach interactions.β’ Create outreach initiatives that will regularly communicate facility mission with referral sources through
phone, email, e-blast, written notes, events, networking and other professional activities. β’ Provide the facility or receiving team with appropriate information to facilitate admission when
necessary. β’ Develop and maintain administrative and clinical operations that provide quality care and outcomes for the patients. β’ Contribute to the planning and implementation of program goals, objectives, personnel, resources and equipment. Implement the approved program plan objectives, timetables, expected outcome and measures for assigned areas of responsibility β’ Develop and implement procedures to ensure efficient operations of the program; continually identify opportunities to improve program's efficiency; coordinate care delivery, work flow; oversee scheduling, registration, discharge and billing processes for the person(s) receiving services according to facility / clinic procedure. β’ Provide staff management to including hiring, development, training, performance management and communication to ensure effective and efficient department operation. β’ Responsible for an assigned caseload if applicable to the program. β’ Develop and implement treatment plans, develop and maintain and audit charts and records for person(s) receiving services. β’ Provide outreach services in area of clinical specialty, as needed. β’ Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.
OTHER FUNCTIONS:β’ Perform other functions and tasks as assigned.
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:β’ High school diploma or equivalent required. Bachelor's degree in Marketing or related health or social
services field preferred. β’ Two or more years' experience in sales and/or marketing or related health or social
services field preferred.
LICENSES/DESIGNATIONS/CERTIFICATIONS:β’ Must have and maintain a valid driver's license, maintain automobile insurance coverage and have
access to an automobile, where facility requires.
β’ RADT preferred, REQUIRED after 6 months of hire
