Program Specialist

SUMMARY OF POSITION:

Reporting to the Divisional Administrator, the Program Specialist will have oversight of MMC Undetectable Program and activities to ensure effective delivery of all key components of the grant (medical, mental health, and the Undetectable case management.

This position will (1) ensure effective delivery of all various components of the program (HIV medical and care coordination, (2) facilitate access to medical care, care coordination, and mental health services by promoting the well-being of HIV infected (virally and not virally suppressed) patients through direct services and advocacy; and (3) manage program content and the collection of data for reporting, as well as the improvement of processes. Work with HIV undetectable and HIV detectable patients and delivery of all key components of the program: medical, mental health, case management and incentive monitoring. Participate in program-wide leadership operations meetings, grants meetings, and care coordination team meetings to liaise with clinical and administrative leadership.
Create and implement high quality services for the program (i.e. case management and care coordination). Complete required grant and internal reports. Ensure compliance with all requirements of and deliverables of the grant.

EDUCATION AND REQUIREMENTS:

Bachelor's Degree is required, Master's Degree is preferred.
A minimum of five years' experience with HIV populations and medical care coordination.

Must have concrete, demonstrable experience with HIV populations, assessments and care coordination.

1. Must have direct experience working in a position/program serving high HIV populations.
2. Ability to work effectively with diverse groups of people, staff, youth and community members.
3. Persuasive and passionate communicator with excellent interpersonal and multidisciplinary project skills.
4. Passion, idealism, integrity, positive attitude, mission-driven, and self-directed.
5. Microsoft Suite (Word, Excel, Power Point) , social media and data tracking platforms.

Bilingual (English/Spanish) preferred.
