Job Description Summary:
Community Health Center, Inc. (CHC), with offices in Connecticut, Colorado and California, is one of the country's most creative and dynamic providers of primary medical, dental, and behavioral health services, and a leader in practice-based research, health professionals training, and use of innovative technologies to advance health and healthcare. CHC is designated as a federally qualified health center and a patient-centered medical home by HRSA, the Joint Commission, and NCQA, respectively. We deliver more than 500,000 patient visits per year from primary care hubs and community clinics across the state of CT, all connected by technology and common standards for quality. We employ several hundred medical, dental, and behavioral health providers who are engaged in practice, teaching, and research. Our Weitzman Institute is devoted to research and practice transformation and is recognized around the country as one of the premier research institutes focused on improving health care and health outcomes for special and vulnerable populations. In addition, the organization has developed three wholly owned subsidiaries from the original pilot developments within the Weitzman Institute: the National Nurse Practitioner Residency and Fellowship Training Consortium (NNPRFTC), the National Institute for Medical Assistant Advancement (NIMAA), and the Community eConsult Network (CeCN).

JOB SUMMARY

The All of Us Program Specialist will work closely with the All of Us Quality Assurance and Training Lead, Principal and Deputy Principal Investigators and is responsible for program operations and oversight. The Program Specialist will work closely with the Quality Assurance and Training Lead to support the team as needed for outreach, engagement, enrollment and retention activities.

The AoURP is an NIH funded national initiative that seeks to enroll one million people living in the United States in a 10-year longitudinal study and takes into account an individual's variability in genes, environment, and lifestyle. The goal of the program is to help researchers learn more about what affects people's health. The program will give new information and tools to researchers. Their discoveries may lead to more personalized care and treatments. The Community Health Center, Inc. was awarded a grant to be part of the Health Provider Organizations that enroll patients into the program.

ROLE AND RESPONSIBILITIES

Duties include, but are not limited to, the following:
  • Having a working knowledge of the All of Us Research Program (AoURP) established infrastructure and its relation to and operations at CHCI
  • Coordinating operational aspects of the AoURP including:
    • Participant Outreach/Enrollment, Engagement and Retention
    • Informed Consent Process
    • Participant information, Biospecimen, and Physical Measure collection
    • Enrollment and Retention site specific needs
    • Developing and piloting new workflows
  • Assisting with the development and administration of assessments for competencies, and trainings
  • Provide oversight and management of the Research Coordinators
  • Responsible for the quality assurance tracking of the All of Us Program components
  • Develop and document effective outreach, engagement, and retention strategies, including leading and participating in clinic-based and community meetings, workgroups and advisory committees.
  • Effectively execute outreach, engagement, enrollment and retention strategies and workflows, and assist with critically evaluating and modifying as needed.
  • Assist patients with all steps of the enrollment process- provide a thorough understanding of the study and answer all questions, obtain informed consent, administer surveys and record data
  • Create internal work plans, dashboards and reports as required to assess productivity and effectiveness of workflows.
  • Evaluate effectiveness of information and activities and recommend changes as warranted.
  • Participate in program required meetings led by the NIH, MITRE and other FQHCs
  • Collaborate and work effectively with members of the CHCI All of Us Research Program team and National partners.
  • Professional responsiveness and adaptability to rapid change.
  • Building constructive and effective relationships both internally and externally.
  • Other duties as assigned by the Principal and Deputy Principal Investigators.
QUALITFICATIONS AND PREFERRED SKILLS
  • Bachelor's degree (Required)
  • Strong interpersonal and verbal communication skills
  • Must be able to travel to all CHCI sites, frequently
  • Must be able to work independently
  • Highly outgoing, friendly, enthusiastic, and energetic
  • Ability to document and track workflows
  • Ability to create written documents to serve as a record of operations
  • Proven ability to be flexible, and to adapt and work in a fast-moving environment
  • Ability to travel locally and out of state as needed
  • Health care experience required
  • Working knowledge of Microsoft Word, Excel, PowerPoint and Outlook
PHYSICAL REQUIREMENTS/WORK ENVIRONMENT
• Must be able to sit for extended periods while working at a computer
• Must be able to carry laptop and recruitment materials as part of recruitment activities
• Must be able to independently travel to health center satellites and participate in recruitment events in the community independently (Eastern or Western Region)

WORK SCHEDULE DEMANDS

  • Occasional mornings, evenings and weekends as needed.
