Community Health Center, Inc. Job Description:

Embedded within one of the largest community health centers in the country, the Weitzman Institute is dedicated to primary care innovation and transformation through research, education, and policy, with a particular focus on the underserved. Since 2007, the Institute has grown into a nationally recognized organization with expertise in health services research, continuing education, and workforce training. Its unique environment, within a safety-net health system, has enhanced its ability to conduct research and promote innovation aimed at improving outcomes, decreasing health disparities, and developing implementation strategies for evidence-based, integrated practice for Federally qualified health centers and other primary care practices nationwide. The Institute has worked with practices in all 50 states, focused on building provider capacity and overcoming provider isolation in 15 content areas highly relevant to primary care, including complex integrated pediatric care, trauma-informed care, pain management, substance use disorders, HIV and HCV treatment, COVID-19 care, LGBT Health, diabetes and health weight management, as well as other complex conditions.

The Weitzman Institute Program Specialist for the Prescription for Play (P4P) program will support an exciting national initiative to educate caregivers about the importance of play on child development in early childhood by engaging pediatric providers in an intervention to educate families in routine well child visits. P4P is a collaborative program between the Weitzman Institute and the LEGO Group, funded by the LEGO Foundation. Reporting to the Program Manager of the P4P program, the Program Specialist will be responsible for strengthening relationships with a set number of providers and clinical partners engaged in the P4P program. The Program Specialist will lead virtual and in-person visits with clinical teams to support the in the implementation and research of the P4P program. The Specialist will share feedback to the P4P project team to help inform program strategy, research initiatives and future growth of the program. The Program Specialist should be comfortable working collaboratively with an interdisciplinary project team, internal and external stakeholders, including researchers, clinicians, leaders in primary care organizations, and private and federal agencies in a fast-paced environment.

GENERAL RESPONSIBILITIES
  • Develop and maintain relationships with clinical sites and pediatric providers engaged in P4P program through frequent communication, including by not limited to virtual check ins, email, and occasionally site visits
  • Support the recruitment and training of new participants (providers, care team staff, clinic leaders)
  • Under the direction of the P4P Program Manager, create and implement quality improvement plans to support sites' efforts to maintain or reach a higher level of program quality
  • Support clinics engaged in P4P research initiatives by coordinating efforts between P4P research team and clinical research sites to help sites successfully meet research milestones, including IRB approval, training providers, communication to key stakeholders at research sites
  • Work with P4P Project Manager to support sites in ordering materials to start or maintain delivery of program
  • Assist assigned sites in completing program evaluation
  • Create frequent P4P Impact Reports for clinical leadership for assigned clinics
  • Provide insights to the P4P project team on how to best support clinics through new trainings and other initiatives
  • Coordinate with P4P Clinical Advisors on opportunities for their engagement with participating sites
  • Support the recruitment and communication with P4P Task Force members
  • Record/maintain minutes from meetings and forward/follow-up with respective personnel according
  • Identify and escalate critical issues in a timely manner
  • Performs other duties as assigned
  • Occasional travel will be required (up to 5 trips per year)

REQUIRED QUALIFICATIONS
  • Bachelor's Degree BA/BS required
  • 1-3 years of work experience, preferably in an health-related or early childhood field
  • Excellent customer service skills to support pediatric providers and their teams
  • Excellent oral and written skills
  • Demonstrated ability to establish positive working relationships in a highly-relational position
  • Computer and technical experience including but not limited to: Microsoft Office and knowledge of web-based platforms and software such as: Office 365, Zoom web conferencing platform, survey management software
  • Self-motivated, independent worker who functions effectively in a virtual work environment, while being an enthusiastic, collaborative team member
  • Ability to prioritize multiple tasks, meet deadlines, and adapt flexibly
  • Ability to use data and quantitative reasoning to analyze trends, opportunities, and issues and contribute innovative ideas to improve
  • Strong organizational skills and attention to detail

PREFERRED SKILLS
  • Education and/or experience within the fields of health care or early childhood related fields
  • Experience with practice change and quality improvement efforts
  • Experience working with diverse populations and underserved communities

  • Ability to work remotely
  • Ability to be flexible in work schedule in order to provide the most effective and efficient program support
