Project Analyst

This position is responsible for the day-to-day implementation, monitoring and evaluation of BAHN Health Home's projects and programmatic initiatives with its BAHN care coordination partner organizations, contracted Medicaid Managed Care Plans, community providers and referral sources. This includes analytics and reporting of programmatic data, in accordance with NYSDOH and CMS policies and requirements.

Three years of experience. Bachelor's Degree required, Master's Degree preferred.

• Strong project management and organizational skills• Understanding of program design, quality assurance and performance improvement, monitoring and evaluation and change management• General understanding of opportunities available with health information technology • Excellent interpersonal skills and ability to work well with staff of various professional backgrounds• This individual ensures that critical operational activities are executed timely and are of high quality• Outstanding oral and written communication skills• Strong proficiency and faculty over Microsoft Office programs, especially Excel, PowerPoint, Visio, Word, Outlook• Strong analytics skills to identify issues and draw out information from large data sets are critical• Prior experience working cross-functionally and with community-based organizations will be helpful• Experience with Health Home or Care Management is highly preferred
