Job Description Summary:
Community Health Center, Inc. (CHC), with offices in Connecticut, Colorado and California, is one of the country's most creative and dynamic providers of primary medical, dental, and behavioral health services, and a leader in practice-based research, health professionals training, and use of innovative technologies to advance health and healthcare. CHC is designated as a federally qualified health center and a patient-centered medical home by HRSA, the Joint Commission, and NCQA, respectively. We deliver more than 600,000 patient visits per year from primary care hubs and community clinics across the state of CT, all connected by technology and common standards for quality. Job Description:

JOB SUMMARY

The Project Manager will be responsible for working collaboratively with team members and stakeholders to ensure that all elements of the Optimizing Virtual Care Initiative of CHC are successfully planned, implemented, evaluated and reported. The Optimizing Virtual Care Initiative of CHCI is a key and prominent new initiative, nationally funded as one of 29 organizations selected to create and innovate with new models of virtual care that advance access and impact health outcomes with a particular focus on improving access to and quality of behavioral health services. The Project Manager will work under the supervision of the Principal Investigator for this initiative.

ROLE AND RESPONSIBILITIES
  • Organization and facilitation of all project meetings including steering committee and workgroups in collaboration with Principal Investigators
  • Ensure timely communication and report submission to funder
  • Project plan development, tracking and communication among steering committee and workgroup members
  • Communication with all stakeholders on a regular basis
  • Development and submission of monthly, interim and final reports
  • Assure appropriate data integrity and data collection for valid outcome reporting and subsequent research, evaluation, and publications
  • Regular communication with selected vendors and monitoring of performance under contracts
  • Other tasks and assignments as indicated
  • Participate in forums, panel discussions and poster presentations as appropriate
QUALIFICATIONS

Required Skills and Education

  • Bachelor's degree BA/BS required preferably in a related field
  • At least 3 years of related work experience
  • Ability to work independently and collaboratively
  • Excellent interpersonal, written and oral communications skills
  • Strong relationship development skills with problem solving techniques
  • Superior organizational skills and attention to detail
  • Proven ability to multi task and meet established deadlines
  • Proficiency in Microsoft Excel, Word, Powerpoint required
  • Training and experience in project management strongly preferred
