Job Description
POSITION SUMMARY:
Provides direct supervision for Social Workers. Accountable for hiring, scheduling, supervising and completing performance appraisal process of all social work staff. Leads efforts to develop, implement, evaluate and modify the care management measuring and monitoring plan. Establishes collaborative relationships within Boston Medical Center, which assist in implementing and evaluating the measuring and monitoring plan.

EDUCATION:
- Masters degree in Social Work required with a minimum of 3-5 years progressive experience in a healthcare setting.
CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:
- Licensure to practice social worker in the state of Massachusetts, required.
KNOWLEDGE AND SKILLS:
- Skills necessary to build credible and effective relationships with and community service agencies.
- Strong interview, assessment, organizational, and problem-solving skills.
- Ability to identify appropriate community resources on assigned caseload and to work collaboratively with patients, families, multidisciplinary team, and community agencies to achieve desired patient outcomes.
- Possesses excellent interpersonal communication and negotiation skills in interactions with patients, families, physicians, and health care team colleagues.
- Possesses strong analytical and PC skills.
- Exposure and/or experience in preacute and postacute care, as well as community resources.
- Ability to work independently as well as to develop collaborative relationships with physicians, families, patients, interdisciplinary team, and other community agencies.
- Demonstrates excellent communication and negotiation skills.
- Demonstrates ability to analyze, develop, and manage change and to integrate continuous quality improvement principles for service and organization work improvements.
- Ability to work with people of all social, economic, and cultural backgrounds; be flexible, open-minded, and adaptable to change.
- Demonstrates the ability to connect patients and families with necessary services, both inside and outside the network.
- Maintains a current working knowledge of services available in the local community, particularly services available to patients with limited or nonexistent payment resources.
- Must demonstrate patience and tact when dealing with patients, families, and other staff.
- Demonstrated expertise in data management and reporting.
- Proficiency in Microsoft Word, Excel, and Access applications, desired.
- Active membership in professional and leadership organizations preferred.
- Analytical abilities necessary to organize, supervise, and evaluate the work of others, to develop, interpret, implement, evaluate policies, procedures, standards, budgets, and to utilize current best practice social service skills
- Subject matter expert demonstrating proficient knowledge of social services, pre/post acute community resources, placement, resource utilization, discharge planning, performance improvement, and managed care reimbursement.
- Strong organizational and time management skills, as evidenced by a capacity to prioritize multiple tasks and role components.
