About This Role:
The QP is responsible for setting up PCA services for clients who qualify for PCA services as determined by their PCA assessment. The QP develops the client care plan with input from the Responsible party and assessment document. They may be asked to assist with medical trainings if needed. They also serve as a resource for the client, RP or PCA on appropriate program use and other services to help meet the health and safety needs of the client in the community. The QP may assist in training, supervision and evaluation of services, as well as ensuring quality assurance of the PCA services at Accra Care. They are expected to stay abreast of current policies and changes to the program, and to participate in committees and meetings to enhance their knowledge of services beneficial to their clients.
**This position requires travel to clients homes around the St. Louis County area**
About You:
Qualified Professional’s must be qualified in at least one of the four following ways:
1). A baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older; OR
2). An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older; OR
3). A diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older; OR
4). A minimum of 50 hours of education and training related to human services and disabilities and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3)
Qualifications also include:
- Willingness to be trained on the services needed by people with disabilities and the elderly in Minnesota.
- Willingness to be trained on home health care requirements, regulations, and protocols
- Demonstrated creativity, critical thinking, and problem-solving skills.
- Able to affect change and demonstrate positive leadership.
- Excellent organizational, problem solving, communication, and interpersonal skills
- Willingness to be trained on federal and state statutes applicable to services for people with disabilities and the elderly in Minnesota.
- Must possess intellectual strength and have a keen understanding of internal systems needed to support services.
- Must pass a background study with the Minnesota Department of Human Services.
- Have a valid driver’s license, clean driving record and reliable transportation to use for work driving purposes
