Job Summary
The Referral Navigator will serve a specialized call-center role, working to stratify patients by need, risk, and/or geographic resources for assistance with a Find Help referral or assignment to a Community Health Worker. This method will focus the support on most time-sensitive requests from our most vulnerable patients and families, while also keeping CHW resources optimized for use with the highest-risk patients. The referral navigators will support referrals from population health, in-patient case management and the medical group for follow-up post-referral. The referral navigators will also connect patients to an Atrium Health primary care practice post referral as well. The Referral Navigator is a key member of the Community Health Acute Social Needs team and recognizes the importance of addressing acute social needs and connecting community members to resources.
Essential Functions
- Manage the intake of social needs referrals and requests for ASN CHW team services
- Establish referral communication strategies for ASN team members
- Assist with internal and community-based organization outreach and relationship building work
- Provide training on the Find Help platform
- Assist with organizing Find Help data and updating user groups
- Help with Find Help technical management for listed ASN team programs
- Manage the CRH group email and respond to inquiries from internal teammates and external community partners
- Communicate with Community Health Worker team members to facilitate client care
- Identify resources for clients to overcome barriers to care and facilitates access to community resources, including locating housing, food, clothing, prenatal classes, parenting, and providers to teach life skills, and relevant mental health services.
- Remain aware of current services offered by service providers, such as mental health, housing, and employment assistance
- Works to reduce cultural and socio-economic barriers between community members and institutions.
Physical Requirements
Light to moderate walking and standing, carrying small to medium sized boxes (no more than 5-10lbs) for occasional community events
Education, Experience and Certifications
- High School Diploma or GED required. Bachelor's degree in a relevant public health or human service field preferred. Strong interpersonal skills, good communication and literacy skills, and demonstrates the ability to develop rapport with community health workers and care teams required.
- Strong understanding of cultural competency with the target population
- Bilingual (English/Spanish) preferred
- Computer literacy desirable
