Regional MDS Nurse - RN Location:Based in Southern California, Travel required Status:Full-Time $110,000–$130,000/year ** RN license and previous regional MDS experience in skilled nursing required ** Ready to lead with purpose and elevate outcomes across skilled nursing facilities? What You’ll Do: • Support and oversee MDS/RAI processes across multiple SNFs • Audit assessments and ensure compliance with federal and state regulations • Mentor and train MDS Coordinators and clinical leaders • Partner with IDT teams to align care plans with best practices • Monitor PDPM and case mix accuracy to support reimbursement goals What You'll Need: • Active RN license (Required) • 2+ years of long-term care experience • 2+ years MDS/RAI experience in a leadership or trainer role • Strong knowledge of MDS 3.0, PDPM, CAAs, and survey prep • Willingness to travel within Southern California (reliable transportation required) Join Madison Creek Partners and help elevate the care, systems, and support that shape outcomes across our skilled nursing facilities.
