Job Description for Registered Nurse
Field Nurse Home Health Care and Summary:
Responsible for providing skilled nursing services for clients of Origin Home Health Care within the parameters of their educational preparation and their demonstrated abilities.
Location/Area Coverage Needed:
Westside Jacksonville
Skills and Abilities of Field NurseHome Health Care:
• Current license in the State of Florida as, Register Nurse.
• Minimum of one year working in a health care field preferably home health care
• Demonstrated ability to document accurately
• Excellent communication skills
• Must be detail-oriented, flexible and organized
• CPR, TB screening, background check, current drivers license
• Works well in a team environment
• Must demonstrate professional behavior and serves as a role model for other employees.
• Ability to assist patient in learning appropriate self care techniques.
• Completes agency competencies upon hire and annuall.
• Strong clinical assessment and critical thinking skills.
• Knowledge of relevant healthcare regulations and standards
Responsibilities of Field Nurse Home Health Care:
• Adheres to all agency, state/federal regulations.
• Demonstrates knowledge and skill of the agency complaint , abuse, neglect and exploitation process, state/federal guidelines and DOH Incident Reporting Policy and Procedure.
• Works under the supervision of a registered nurse to carry out the plan of care.
• Provides teaching to client/caregiver to promote self care activities.
• Work only within the scope of his/her license or proven competency.
• Complies with State and Federal Regulations.
• Documentation of patient care activities timely and accurately.
• Provides skilled nursing services and specialized nursing procedures under the direction of the physician or registered nurse.
• Prepares equipment and materials for treatments, observing sterile and aseptic technique as required RN case manager in adhering to the plan of care.
• Provides leadership by planning, developing and organizing, and implementing.
• Evaluating and directing the activities of the Administrative support personnel.
• Develops and maintains written operational policies and procedures in conjunction with other partners and personnel.
• Establishes a rapport with all employees.
• Interprets/explains the agency’s policies and procedures to employees, clients, family members, and government agencies.
• Promotes an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment.
• Maintains a good public relations program that serves the best interest of both agency and the community.
• Conduct in-home assessments of patients' healthcare needs.
• Develop and implement individualized care plans for patients.
• Administer medications and provide skilled nursing care.
• Monitor and assess patients' conditions and report changes to the healthcare team.
• Educate patients and families on proper healthcare practices and self-care.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Maintain accurate and timely documentation of patient care activities.

*Must have 1+ years of Home Health Experience.

Pay: $78,000.00 - $90,000.00 per year
