Registered Nurse Case Manager (RN) (Bilingual)

Core Responsibilities: • Conduct assessments of incoming patients • Manage your own case load and conduct visits where patient resides • Provide symptom management and oversee patient’s plan or care while collaborating with Hospice team members • Participate in anon-call rotation for evenings and weekends • Have knowledge and understanding of all services hospice provides and keep patient and family informed of all benefits available Day to Day Activities: • Educate patient and family on disease progression, make visits, care coordination and management of patient and family needs related to Hospice diagnosis • Deliver outstanding bedside care • Foster relationships with your patients and families to make a difference in their greatest time of need Who the Job reports to: • Director of Clinical Services Requirements/ Qualifications: • Bilingual (Spanish) • Valid Registered Nurse (RN) license • 1-2 years of nursing experience (preferred) • Driver’s license, insurance and vehicle • Utmost compassion and integrity Job Type: Full-time Pay: $80,000.00 - $85,000.00 per year Benefits: • 401(k) • 401(k) matching • Dental insurance • Flexible spending account • Health insurance • Life insurance • Paid time off • Tuition reimbursement • Vision insurance Experience: • Hospice: 2 years (Preferred) • Home care: 2 years (Preferred) Language: • Spanish and English (Required) License/Certification: • RN License (Required) • Driver's License (Required) Work Location: On the road
