Our hospital serves 2.5 million people within 9 counties, averaging a daily census of 641 patients. We're affiliated with UCSF as a teaching facility; trained 3,000 Medical Doctor's to date plus continuous invests in research and training grants.

Responsibilities:

Coordinates all aspects of the Stroke Program under the direction of the RN Program supervisor to achieve and maintain the Joint Commission Certification for Community Medical Centers (CMC). Coordinates stroke patient care across the continuum in collaboration with unit leadership, providing stroke-related training and competencies for staff. Coordinates community outreach. Supports monthly interdisciplinary meetings, quality assurance and performance improvement activities for the stroke program.

Accountabilities

1. Serves as the program coordinator for the Stroke Program at all CMC facilities including those departments directly involved in the Stroke Center: Emergency Department, Surgical Services, Critical Care units, 9-West Neuroscience Unit, General Medical-Surgical Departments, Rehabilitation Services, Out-Patient follow-up and other areas as indicated.
2. Identifies all stroke admissions and tracks in the Stroke Registry.
3. Implements performance measures and collects data to evaluate effectiveness of treatment and monitors clinical targets vs actual performance to identify variance from the clinical practice guidelines. Maintains the integrity and security of data and information.
4. Supports and mentors staff as indicated in the process for identifying, reporting, managing and tracking unanticipated, untoward adverse events related to stroke patient care.
5. Supports clinical operations as they relate to stroke patient care. Reports unresolved problems to leadership. Facilitates patient experience by ensuring patient/family concerns are addressed.
6. Communicates and supports stroke and organizational initiatives.
7. Applies advanced knowledge in the clinical area setting as a role model and resource. This includes maintaining up-to-date knowledge of clinical and technological advances in the area of stroke patient care.
8. Communicates educational and competency needs of clinical staff to the RN Program Supervisor. Provides clinical education and concurrent learning to develop stroke nursing and related skills. Provides learning opportunities through on-the-job experiences. Maintains readily available reference/resource materials. Assists in the tracking of applicable staff competencies.
9. Ensures compliance with the Certification Participation Requirements and the Standards and Elements of Performance from The Joint Commission’s Disease-Specific Care manual. Maintains survey readiness and remains up-to-date on regulatory standards and clinical practice guidelines. Coordinates survey responses, working closely with surveyors to provide information and assistance appropriately for the Stroke Program.
10. Collaborates with the Unit Manager/designee to update staff on changes in Stroke policies, Clinical Practice Guidelines and orients new staff to Stroke resources.
11. Participates in community outreach programs for targeted neuroscience populations and works together with other Clinical Program Coordinators to provide Stroke outreach.
12. Coordinates with the department staff and case managers, as indicated for appropriate rehabilitation, palliative care, placement and/or outpatient follow up.
13. Performs other duties as assigned.

Qualifications: Education• High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required• Bachelor's Degree in Nursing or Healthcare related field preferred Experience• 2 years of relevant experience in the healthcare industry or a healthcare setting required Licenses and Certifications• RN - Current and valid Registered Nurse license to work within the state of California required• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required• One of the following is required within 1 year of hire or transfer into positiono SCRN - Stroke Certified Nurseo CNRN - Certified Neuroscience Nurseo ASC - Advanced Stroke Coordinator
