Job Description

TITLE: REGISTERED NURSE REQUIREMENTS:R.N. licensed to practice in NYS. 2 years experience in delivery of community based health services to the persons with intellectual disability I/DD preferred. The quality of care and life in the facility should be of first importance to the nurse. ACCOUNTABILITY: Reports to the Residential Coordiantor and Nurse Supervisor. DUTIES AND RESPONSIBILITIES: 1. The Registered Nurse will: • Oversee the ongoing medical care of the consumers. • Correspond with physicians on as-needed basis. • Participate in quarterly/ semi- annual and conferences on all consumers. • Periodically re-evaluate the type, extent, and quality of services and programming of each individual. • Develop admission and/ or discharge plans. • Make referrals to appropriate community resources • Periodically attend staff meetings at each residence as needed • Conduct quarterly or semi- annual and annual reviews/evaluations of each consumer. • Assess each consumer’s ability to administer medications annually or semi annually as needed. 2. Conducts in-service training in: • Personal hygiene • detecting signs of illness • Control of communicable diseases • Nutrition and weight control • Family life • Sex education (including family planning and venereal disease counseling) • basic nursing skills, i.e. –blood pressure, drug side effects, height and weight, dietary needs • first aid and all other health areas as needed 3. Control of communicable diseases and infections through: • Identification and assessment • Reporting to medical authorities • Implementing of appropriate protective and preventative measures 4. Develops a written care plan for each resident who demonstrates a chronic/ severe/ ongoing medical problem and may require ongoing medical attention as a part of the total habilitation program. Monthly progress notes to be kept on these clients; modification of the written nursing plan as needed. 5. The registered nurse shall oversee administration of all medication 6. The registered nurse shall be responsible for conducting appropriate follow ups on all medical emergencies that may occur in the residence. He/she will be responsible for reading the residence communication books, incident reports requiring medical attention, and medical record sheets. 7. The registered nurse shall be responsible for having a general knowledge of all clients’ medical history/needs. NURSE PARTICIPATION IN STAFF MEETINGS:The R.N. will attend staff meetings on as needed basis in order to provide in-services training, discuss special medical problems, and act in the capacity of a liaison with various community resources. PROCEDURE ON STAFF TRAINING: 1. The nurse will develop written curriculum to cover the in-services training needs outlined above. 2. The nurse will conduct in-service sessions for direct care staff. He/she will keep records of dates of sessions and will discuss effectiveness of course with individual residence supervisors. 3. The nurse will pinpoint need for further in-service training. If training cannot be provided by facility staff, the nurse will notify the Supervisor of the need for outside resource that can provide the training. POLICY: A.M.A.P. Certification (Approved Medication Administration Procedure) The registered nurse shall oversee the administration and re-ordering of all prescription drugs dispensed; in the residence. The residence Supervisor will be responsible for sending staff to OMRDD approved AMAP courses, and the nurse shall: 1. Administer Practicum part of AMAP training to staff 2. Do annual re-certification of AMAP personnel 3. Check medication charts on a regular basis and sign off on sheets each month 4. Do periodic re-assessments/training of AMAP staff as needed The R.N. will carry out other duties as assigned/required for the health and well being of the individuals we serve.
