Registered Nurse (RN)-Home Health-Streeterville & Surrounding Areas

Luxe Home Health is looking for a dedicated Home Health Registered Nurse (RN) to join our team. In this role, the Home Health Registered Nurse (RN) will provide compassionate, high-quality care to patients in their homes. This is a flexible, per diem opportunity ideal for an experienced RN with a passion for home health care. Responsibilities: • Perform comprehensive patient assessments and develop individualized care plans. • Administer prescribed medications and treatments. • Monitor patient progress and communicate with the healthcare team. • Provide patient and family education regarding healthcare needs. • Complete accurate and timely documentation for each visit. • Travel throughout the service area to provide patient care. Requirements: • A valid IL Registered Nurse (RN) license is required. • At least 2 years of nursing experience, preferably in home health. • CPR and BLS certifications are required. • A valid IL driver’s license, auto insurance, reliable transportation, and the ability to travel throughout the service area are essential. • Axxess experience is preferred.
