Remote Primary Care Coordinator

The Opportunity
Pine Park Health is hiring a Remote Primary Care Coordinator to help us care for patients using strong customer service and patient coordination skills. This role requires a medical assistant certification, preferably with front and back office experience.
In this role you will work closely with providers and the clinical team to help patients navigate their care, from appointments to insurance verification, you will be key in helping patients understand and access care.
What you’ll do
- Hours: Typical M-F, 8:30am-5pm, with the opportunity to add on 5-7pm or weekend shifts
- Location: This is a remote-first role
- You’ll be the primary partner to the Provider(s) you are assigned to:
- You will collaborate on delivering excellent patient care and experience as needed
- You’ll get to know the patients in your panel quite intimately over time, and you will be able to not only respond to their needs but also anticipate them ahead of time
- Prepare the patient for their appointments including but not limited to obtaining accurate history of present and past illness, coordination and communicating with the patient's Durable Power of Attorney (DPOA), families, and communities etc.Â
- Schedule patients for medical appointments with providers outside speciality offices, document encounters in the EMR, monitor appointment attendance, and inform patient, family, and team appropriately.Â
- You will manage all prior authorizations, obtaining medical records, gathering medical history and context via phone calls, patient scheduling, managing referrals, patient transitions , etc.
- Answer Pine Park Health telephone and address patients and providers issues, relaying urgent matters to the Clinical Operations Lead.
- Clinical documentation: Ensure that every activity is always documented in the patient chart
- Only act within the scope of his or her authority to practice.Â
- Other duties as assigned.Â
- Timely completion of all mandated training and education.Â
- Timely complete completion of all mandated occupational health screenings as needed.Â
What we’re looking for
- Must reside in CA, TX, FL, WA, CO, AZ, ID, OR, NV, UT; preference given to those residing in TX, UT, and ID
- Front and back office experience:Â prior authorizations, obtaining medical records, gathering medical history and context via phone calls, patient scheduling, managing referrals, etc.
- Strong written and verbal skills: Able to convey a message clearly and concisely to patients and providers
- Urgent care mindset: The ability to work quickly and think two steps ahead
- Documentation: The ability to properly document in patient charts to ensure accurate and effective documentation.Â
- Technology: Comfortable and adaptable, enjoys working with new technologies and platforms (e.g. Elation, Slack, Monday.com)
- Experience working remotely and independently without constant support
- Ability to think quickly andÂ
- Commitment to support and encourage our most vulnerable seniors to their full potential

- Educational requirements:
- High School Diploma or Equivalency
- Basic understanding of clinical and back office experience in a primary care settingÂ
- Certification/Licenses Required:Â
- Certification by a Medical Assistant Certifying Organizations approved by the CA Medical Board, preferred.Â
