Registered Nurse - CHHA Must have experience with Oasis documentation. Wound Care experience is a must Practice independently in the community as part of an interdisciplinary care team. Deliver personalized nursing and care management to patients in their home or care facilities. Constantly evaluate evolving patient needs and respond with plan of care adjustments. Qualifications • Current license to practice as a Registered Nurse in New York State • Must have CHHA and Oasis experience • Valid driver's license or NYS Non-Driver photo ID card may be required #CM-3
