This position works with, supervises and evaluates clinical field nursing staff members in the home health program of care. The position also coordinates care with other clinical management team members for PT, OT, speech therapy, etc. The Home Health Clinical Manager is also responsible to: • Ensure that client care meets Federal/State regulations and Agency standards for documentation and clinical accountability. • Act as liaison between clinical field staff, client, physician, family members, and other client-appointed individuals, and reports all information to those involved in the care of the client in a timely manner. • Ensure that clinical and agency updates on all current care standards are initiated and maintained and ensures staff is updated on these standards. • Directly evaluate and oversee all Home Health intakes, admissions and ongoing care. Qualifications: • Min 1 year experience as home health field nurse • 1 year of supervisory experience as an RN in a Home Care Agency preferred. • RN Graduate of an approved school of nursing, or higher degree. • Bachelor’s Degree preferred • Current Professional RN License. • Current CPR, TB, Fingerprint Card (If you do not have these available, let us assist you in obtaining these). • Be knowledgeable of the statutory and administrative rules of State and Federal Home Care guidelines to ensure compliance within the agency.
