Medically Home operates a decentralized care model for patients with serious, complex, or high-risk illnesses in partnership with health systems to safely care for their patients at home, across the care continuum, including hospital-level care. Medically Home provides health systems with all that is needed to safely care for patients, including the clinical protocols, reimbursement model, platform technology, and fulfillment of all the clinical services required in the home through partners. Clinicians and patients broadly prefer this model over traditional care; both clinical and financial outcomes are improved as compared to traditional care.

 

Job Summary:

MH partners with clients to enable high quality acute hospital care in the home. One important aspect of this care is building and managing a network of in-home services (“Rapid Response Services”) that partner with the virtual hospital clinical team to provide high quality patient care. As our customers mature and grow their virtual hospital programs, MH grows their Rapid Response Services network to support. MH is seeking a RRS Implementation Associate to provide support for customer program expansions, assist in developing and maintaining implementation tools to standardize processes. Must be willing to do extensive traveling to client’s sites. They will ensure that new processes and software are implemented effectively, testing the workflows and systems to ensure they meet customer needs, and support training.

Key Duties and Responsibilities:

  • The successful candidate will be a flexible team player that can provide structure and organization while allowing space for the unique needs of each implementation.

This includes:

    • Collaborate with internal and external subject matter experts to gain full understanding of project needs and additional resources that may be required.
    • Create and maintain a project timeline and plan.
    • Execute project plan segments, including design of workflows, sourcing of rapid response partners, training of clients and partners, and go-live support.
  • With a firm understanding of the MH model, will be able to present analysis and recommendations to internal teams and clients.
  • Proactively identify issues; troubleshoot and facilitate issue resolution.
  • Manage upward by communicating progress and challenges, requesting guidance when needed, presenting potential solutions, and identifying next steps.
  • Communicate effectively with different levels regarding information needs. The associate will be responsible for leading broad work groups of customer and internal stakeholders (clinical, operational, and technology) to inform the overall project key milestones.
  • Work with MH Implementation and rapid response teams to help identify best practices and standardize tools and processes for future implementations.

 

Requirements and Qualifications:

  • Bachelor’s Degree.
  • Ability to travel as needed. Estimated up to 30% Travel (generally Monday – Thursday with the ability to fluctuate based on customer need and implementation cycle).
  • 2+ years’ experience with integrating and interfacing major healthcare systems and services, including analysis, gathering of customer requirements, product, and training.
  • Must be comfortable working in a fast-paced environment with multiple competing priorities.
  • Strong written and verbal communication skills.
  • Good organization skills and ability to multitask.
  • Good problem solving and analytical skills.
  • Exceptional customer service skills.
  • Outstanding communication skills: meaning you understand the significance and appropriate use of tone and mediums based on circumstance and audience.
  • Excel, Visio (or other workflow tool) and PowerPoint skills required.
