Job Description

Medically Home operates a decentralized care model for patients with serious, complex or high-risk illnesses in partnership with health systems to safely care for their patients at home, across the care continuum, including hospital-level care. Medically Home provides health systems with all that is needed to safely care for patients, including the clinical protocols, reimbursement model, platform technology, and fulfillment of all the clinical services required in the home through partners. Clinicians and patients broadly prefer this model over traditional care; both clinical and financial outcomes are improved as compared to traditional care.

Position Description:

As the company establishes partnerships with providers on a national scale, Medically Home is seeking an exceptional Rapid Response Services (RRS) Operations Network Director to support the in­ home delivery of care. Reporting to the RRS Regional Market Director (RD), this director will own the Acute Rapid Response ecosystem to create efficiency and scale in support of Medically Home's national growth objectives. This person will work closely with the clinical team to determine the supply chain and logistics configuration that can best enable clinicians to provide an integrated, safe, and effective episode of care. In partnership with the RRS Operational Network Director and the RRS RD, fosters and maintains relationships with both internal and external stakeholders with respect to clinical suite of services.

Principle Roles and Responsibilities:

  • Primary focus on the Medically Home growth, implementation, and service partner relationship management

  • In a dyad leadership structure and in partnership with the clinical network director, this individual is responsible for the quality, outcomes, and cost of the in home service network.

  • Achieves key patient care, financial and/or operational outcomes, as found in annual work plan.

  • Leverages and adheres to systems and processes that drive accountability for patient care, financial and operational outcomes.

  • Overall is responsible for (i) the initial implementation (including contracting and operational workflow development) of Rapid Response Services partners in the Acute Rapid Response network of service providers (e.g., Infusion Services, Skilled Nursing, Phlebotomy, Mobile Diagnostics, Courier Services, Meals, DME), (ii) supporting all payor-required credentialing activities of the Rapid Response Services partners for individual markets and (iii) the ongoing maintenance (e.g., adding, dropping, and supporting partners) of the network in the relevant geography.

  • Negotiates vendor service agreements to ensure that all SLAs, costs, and patient quality expectations are met.

  • Liaison between Medically Home clinical team and service partners ensuring that highly collaborative, high-quality care is provided to each patient.

  • Acts as a first-line resource to manage the relationship interface between Medically Home and service partners pertaining to supply, logistics, quality, reporting, credentialing, and training.

  • Serve as the in-market primary interface with each service provider and service partner to ensure adherence to operational service delivery requirements and drive continuous improvement.

  • Work closely with the medical command center’s clinical, implementation, and operational team to ensure seamless delivery of patient care and address any challenges, do root-cause analysis of issues, and drive continuous improvement.

  • Lead on -site training efforts in-market with all Medically Home service providers and supplier partners.

  • Lead in-market initial and ongoing service provider and supplier partners qualifications.

  • Report on supply chain and logistic metrics to enable process improvements and root cause analyses.

  • Identify issues and or risks with meeting clinical and non-clinical supply requirements and implement solutions to meet service level requirements with superior patient experiences.

  • Participate in market-based quarterly Rapid Response Services performance meetings; collect all relevant data collection to support quarterly meetings.

  • Develop and maintain in-market supplies and equipment inventory to ensure efficient resource deployment and utilization.

  • Provide support to member so the Medically Home team related to Rapid Response Services with special projects as required.

  • Independent problem-solving skills with close attention to detail


Experience:

  • 5-8 years professional experience required

  • Health care supply chain experience is preferred

  • Bachelor’s degree required; Master’s degree preferred
