SC NP/PA CF Coordinator

The Pulmonary Cystic Fibrosis (CF) APP Coordinator functions as a member of the Pulmonary multidisciplinary team in the delivery of comprehensive care to patients and families, including, but not limited to those diagnosed with Cystic Fibrosis. The Nurse Practitioner/Physician Assistant functions in collaborative practice with physician(s) in both the clinic and hospital settings. Autonomously performs patient assessment, diagnosis, and intervention according to approved protocols. Develops and maintains own volume of patients under the supervision of a physician. Prescribes a variety of modalities to meet wellness and illness needs, including prescriptive authority. The SC APP Pulmonary CF Coordinator's responsibility to patient care extends from admission to discharge and subsequent follow-up visits/home communication. Under the direction of a board certified pulmonologist, the SC Pulm CF Coordinator collaborates with the multi-disciplinary team to provide patient care, care coordination and education. Primarily providing services to patients 0-21 years, including neonates. Also provides services to patients over 21 years of age in the adult CF population. This position also participates in the CF Foundation re- accreditation including assisting with required document submission and coordination of affiliate center site visit. Assist program director in completion of annual grant application, including but not limited to updates of team information, progress since previous year, goals for coming year, and QI activities. Provide support and guidance for affiliate center staff including scheduling reverse site visits and regular meetings.

Education & Experience

Advanced Practice Registered Nurses • Master's Degree in Nursing as Advanced Practice Registered Nurse • Minimum 2 years of Pediatric Nursing experience or related specialized area strongly preferred

Physician Assistants • Master's Degree in Physician Assistant Studies • Minimum 1 year of experience in related specialized area strongly preferred

Licensure, Registration, and/or Certification

Advanced Practice Registered Nurses: Current Texas professional registered nurse license. Current Texas professional advance practice registered nurse license. Certified as a PNP or FNP. FNP Preferred. Current Basic Life Support (CPR) required. Current Pediatric Advanced Life Support (PALS) required. OR Physician Assistants: Current Texas professional physician assistant license. Certified as a PA-C. Current Basic Life Support (CPR) required. Current Pediatric Advanced Life Support (PALS) required.
