Job Description

Centrica Care Navigators, Formerly Hospice Care of Southwest Michigan, is excited to add another Scheduler to our growing team! This position works primarily Monday – Friday, 8:00am - 5:00pm, at our Paw Paw office. Our schedulers coordinate and maintain the schedules for all homecare clients and personnel and are included in weekend and holiday rotation (approximately every other to third weekend and about 3-4 holidays per year).

Specific responsibilities of this position include:

  • Creating, entering, and maintaining home care schedules for all disciplines in the electronic medical record. 
  • Scheduling client visits according to care plans and staff availability.
  • Providing backup support for provider scheduler and maintaining home care physician visits as needed.
  • Scheduling home care supervisor/staff observation visits.
  • Providing staffing for call-ins, PTO, and long-term leaves.
  • Rescheduling declined visits, missed visits, and reassigned visits.
  • Ensuring continuity of caregiver whenever possible.
  • Acting as a liaison between clients and staff related to scheduling.
  • Contacting care providers regarding day-to-day changes and contacting clients as needed.
  • Maintaining staff and client statistics and running necessary reports (e.g. staff productivity, case count, staff call-in report, missed visit report).
  • Answering all staff scheduling-related calls and referring clinical questions to the appropriate Home Care Manager.
  • Performing general office duties including but not limited to typing, faxing, filing, etc.
  • Providing backup phone assistance on a rotational basis when the Receptionist is absent.
  • Assigning bereavement clients to grief support counselors.

Requirements for this position include:

  • High School graduate or equivalent.
  • A minimum of one-year previous administrative support experience
  • Medical or previous scheduling experience preferred
  • Excellent written and oral communication skills
  • Excellent organizational skills
  • Strong attention to detail and customer service orientation
  • Good keyboarding skills with a high level of accuracy
  • Strong computer skills to include Microsoft Office products and electronic medical record
  • Sensitivity for the confidentiality of materials/information processed
  • Presents a negative TB skin test and/or chest X-ray
  • Remains up-to-date on all required routine immunizations required by LARA and agency policy. Must be fully vaccinated against COVID-19.
