About the role

Our Senior Care Navigator will be the ultimate guide to Octave for our prospective clients.  This role will contribute to Octave’s mission of expanding access by introducing clients to our innovative practice, welcoming new clients to Octave, and guiding them through the behavioral health landscape. In addition to the core responsibilities of the Care Navigator role, they will be involved in interacting with our highest-needs clients, ensuring they receive the support in their time of need. The role will report into, and work closely with, the Director of Clinical Operations. 

The ideal candidate is a strong team player and innovative problem-solver, with a passion for helping people navigate healthcare and an operationally astute eye. You are self-aware, performance driven, and thrive off of solving tough problems. You have mastered the art of communication, executing on projects, and being a thought leader. You enjoy keeping busy and gain energy from working with clients, and internal stakeholders.


Accountabilities:
  • Conduct proactive outreach to understand higher risk client’s needs, and take action when necessary with the support of clinical staff.
  • Participate in initiatives/projects to identify, scope, and implement opportunities for increased efficiencies. 
  • Become a subject matter expert in our tools and workstreams to iterate and improve them as we grow and scale. 
  • Support clients along their care journey, from entry into our system, all the way to their first appointment with their provider.

You’ll Be Set Up for Success if You Have:
  • 3-5 years experience working in a Care Navigation/Coordination capacity, specifically within the mental health or healthcare arena
  • 1-3 years supporting cross-functional initiatives with key stakeholders across the organization
  • Bachelor's degree required, master's level mental health degree preferred
  • Experience with telephonic crisis intervention
  • Excellent verbal and written communication and can adapt your approach to multiple groups
  • Detail oriented and impeccable organizational skills are a must
  • Self-starter, willing to roll up your sleeves and be proactive and solution oriented
  • Nimble and open to frequent change, iterative processes, and operating in a testing culture
  • Understanding of the mental health industry to leverage insights for decision making
  • Must be legally authorized to work in the United States without the need for employer sponsorship now or anytime in the future
