Job Overview

Under the direct supervision of the Manager of Behavioral Health Services, the primary function of the Case Manager (CM) is to provide referral management for Substance Abuse and Behavioral Health patient population. The CM I will follow the program’s Scope of Work to achieve the program goals through the screening of patients, referral organization, referral management, appropriate linkage of care and record keeping.

Minimum Requirements

1. Bachelor’s degree in Social Services, Sociology, Human Services, Counseling or a minimum of 5 years working in health care assisting patients with case and referral management and linkage to care.

2. Current BLS certificate required.

3. Bilingual English/Spanish or English and other predominant language of population served preferred.

Salary Range

$24.00 - $30.00 hourly
