Job Description
Essential Duties and Responsibilities:Responsible for performing clinical and professional pharmaceutical work in the review, processing of prescribed medications, drugs, and supplies needed for patient care.
• Completes patient/client records, charts, including patient assessment, medication profiles, care plans, initial opening patient notes and continuing patient progress notes.
• Completes patient counseling in compliance with clinical program protocols and adhere to and counsel patients in compliance with REMS programs and other programs; Documents any and all interactions with patients and prescribers in the appropriate systems in compliance with protocol.
• Reviews patient demographics and prescriptions issued by physician, or other authorized prescriber to assure appropriate therapy for the patient, accuracy of dose and determine formulas, ingredients, and medications/administration supplies needed for optimal patient outcomes;
• Reviews and/or takes orders from doctor's offices or other authorized prescribers; Validates technician data entry to ensure accuracy of prescription; Answers questions and provides information to pharmacy clients/patients and employees on drug interactions, side effects, dosage, administration and storage of pharmaceuticals and supplies.

• Participates in the coordination of care to meet patient outcomes/goals including, initial referral, medication review and selection, assessment of medication delivery system, coordinated care conferences, patient care plans, compliance programs and discharge of patients from services.
• Works efficiently and in a timely manner to ensure all work is complete before end of day and supervises work flow
• Performs On-Call activities: such as mandatory on call phone duty and in the event of a patient emergency will come in after hours to open pharmacy, as required.
• Adheres to all pharmacy, state, and federal laws and regulations.
• Obtains and maintains minimum of two more pharmacist’s licenses in other states in addition to the existing licenses
• Performs additional assigned tasks related accreditation requirements and other tasks related to the business
• Performs other duties as assigned.
