Staff Pharmacist (30 hours/ Week) - Yonkers, NY

Job Summary We are seeking a dedicated and knowledgeable Staff Pharmacist to join our team. The ideal candidate will have a strong background in clinical pharmacy and patient care, with experience in various pharmacy settings including LTC (Long-Term Care) and acute care environments. This role involves providing comprehensive pharmaceutical care, managing medication therapy, and ensuring the safe and effective use of medications for patients. Responsibilities • Provide expert pharmaceutical care to patients, ensuring optimal medication management and patient safety. • Conduct patient assessments to evaluate medication needs and outcomes. • Collaborate with healthcare providers to develop and implement individualized medication plans.. • Administer immunizations and manage patient education regarding medications and health conditions. • Maintain accurate inventory management of pharmaceuticals and supplies. • Utilize pharmacy software, EMR systems, and EHR systems for efficient documentation and patient record management. • Participate in pain management strategies and critical care initiatives as needed. • Ensure compliance with HIPAA regulations and maintain confidentiality of patient information. • Provide guidance and training to pharmacy technicians as necessary. Requirements • Doctor of Pharmacy (PharmD) degree from an accredited institution. • Active pharmacist license in the state of practice. • Strong clinical pharmacy experience with knowledge of medical terminology, anatomy, physiology, and pharmacology. • Excellent patient service skills with a focus on patient care and assessment. • Familiarity with compounding medications and aseptic techniques is essential. • Ability to manage inventory effectively while ensuring compliance with regulatory standards. • Strong communication skills for effective collaboration with healthcare teams.
