Supervisor, CME Care Coordinator
Job Description
Crossroads Health provides a continuum of quality life-changing behavioral health and specialized chemical dependency treatment services to children, adolescents, young adults, adults and families. As a leader in the behavioral health field, we offer a vast array of programs and services that evaluate, educate, strengthen and support thousands of children, adults and their families each year.
The CME Care Coordinator Supervisor provides supervision to the Care Coordinators. This may include oversight of maintenance of clinical records, outreach, clinical and administrative supervision, and general oversight for daily operates of the program. This individual also participates in treatment team meetings and staffing linking the program to other facets of the agency to ensure coordinator of quality care.
CME Care Coordinator Supervisor Job Duties:
- Accept referrals and assigns Care Coordinators to cases ensuring that youth and families/caregivers have a voice and choice regarding the assignment
- Monitoring and managing care coordinator caseloads
- Provide administrative supervision to care coordinators
- Participation in High Fidelity wraparound team meetings
- ·Participate in agency and community trainings
- Outreach to natural and clinical supports in the community and advocacy for multisystem youth and families
- Attend training, as mandated, with OhioRISE, the Center of Excellence, and those related to health equity/health disparities and cultural competence, trauma-informed care and leadership development.
CME Care Coordinator Supervisor Educational and Experience Requirements:
- Bachelor’s degree in a related health and human services field.
- At least three years’ experience in children’s behavioral health, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field, providing community-based services to children and youth, and their families or caregivers.
