Job Description:

Job Summary: The TCHATT Team Lead provides direct patient care services, which includes assessment care in a way that promotes positive treatment outcomes and meets the needs of our behavioral health patients. This job also provides an active, ongoing program of performance improvement within the department.

Typical Duties:

1. Assists with leader functions such as assessment of qualifications, skills and competencies, provides orientation and training, and provides input into performance review and hiring selections.
2. Serves as preceptor for new employees, providing guidance and education as needed.
3. Provides clients with self-management skills and educational information ensuring full participation in their own treatment and recovery.
4. Coordinates the care of an identified behavioral health population throughout the healthcare continuum.
5. Acts as part of a multidisciplinary team and contributes to the development of a comprehensive plan of care based on a systematic assessment of the physiological and psychosocial needs of the patient.
6. Assists the treatment team in recognizing, treating and managing mental health and psychosocial issues.
7. Communicates and collaborates with patient and/or family to ensure understanding of treatment plan.
8. Monitors patient progress towards established goals and evaluates and modifies plans of care as needed.
9. Documents treatment goals and plans in the electronic medical record of which includes all assessments and all relevant clinical documentation.
10. Utilizes behavioral health expertise to promote change and improve behavioral and physical health conditions.
11. Provides care-management skills and knowledge of local resources for outside referrals.
12. Advises the care team about suitability of clients' settings, i.e. other mental health facilities, specialties, etc.
13. Assists in the process for any recommendations that need required authorizations prior to arranging for treatment.
14. Maintains a positive, collaborative relationship with other departments within the hospital district.
15. Performs other job related duties as assigned.

Qualifications:

Required Education and Experience:
• Master's Degree in Counseling or Social Work.
• 2 plus years' experience working as a Counselor, Social Worker, or similar capacity.
Required Licensure/Certification/Specialized Training:
• Must have Must have current licensure from Texas in one of the following: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor(LPC) from the Texas State Board of Social Work Examiners or the Texas State Board of Examiners of Professional Counselors.
• Certification in Non-Aggressive Crisis Management; must be obtained while in orientation and maintained throughout employment.
• BCLS certification required but may be obtained subsequent to hire.
Preferred Education and Experience:
• 1 plus years of experience leading, supervising and/or managing experience.
• 2 plus years of prior supervisory/management experience.
Preferred Licensure/Certification/Specialized Training:
• Licensed Clinical Social Worker (LCSW)
