- Shift: 3x12s 630am-7pm (shifts will be scheduled per unit needs, Monday-Friday. 36 hours/week).
- Is on-call REQ? yes, 8-9 call shifts per month, including one weekend
- Holiday requirements: Holiday call may be required
- Years of experience REQ: 2
- Experience required: Must be able to scrub complex vascular cases
- First-timers accepted: Not preferred
- Job Requirements
- Certs REQ: NBSTSA and BLS
- Required: Radiation badge
- Open to accommodating block schedule? Requires manager's approval upon request
- What is the local radius rule? 60 miles
- Locals accepted: Yes, on a case by case basis at 10% less than the posted bill rate
- RTO Restrictions: all RTO must be provided at the time of submission. Approvals based on dept needs
- Facility able to call off traveler 1 shift per 2 weeks (pay period)

- Facility would like a copy of all certifications that traveler possess.
- Additional Details
- Qualivis: Attestation required confirming work eligibility in the US and that Agency Staff does have a completed and accurate I-9 form on file.
- Copy of Driver's License Required
- Copy of Social Security Card is required.
- AHA only. RQI acceptable.
