Qualifications: Valid state AZ licensure as a Physical Therapist Home Health PT experience preferred HCHB charting experience is a plus Responsibilities: Conduct comprehensive assessments of patients' physical conditions. • Develop and implement individualized treatment plans based on assessment findings. • Provide hands-on therapy and exercises to promote mobility, strength, and function. • Educate patients and their families on therapeutic exercises and activities. • Collaborate with other healthcare professionals, including nurses, physicians, and social workers, to ensure coordinated care. • Document patient progress, treatment plans, and outcomes accurately and timely. • Monitor and adjust treatment plans as needed to achieve optimal patient outcomes. • Maintain compliance with all relevant regulations and standards.
