Job Description

State License Verification required within 30 days of start
NURSYS Verification (RN, LPN/LVN, AP) required
IV Certification (LVN/LPN/Other) required for LPNs in state of MO or accepted by state of MO
Primary Source Verification required within 30 days of start

Drivers License/State ID required
ACLS - Advanced Cardiac Life Support (AHA only, RQI acceptable) required for RRTs
BLS - Basic Life Support (AHA only, RQI acceptable) required
PALS - Pediatric Advanced Life Support (AHA only, RQI acceptable) required
HIPAA Safety Manual Acknowledgement required annually
Acknowledgement of Pass/Fail Testing required prior to start for specific units
Medication Test/Pharmacology Competency Exam required as applicable by Profession
Competency Exam required as applicable to Profession/Specialty
Mosaic Life Care - Confidentiality Agreement must be reviewed and signed
Mosaic Life Care - Code of Conduct and Acknowledgement must be reviewed and signed
Mosaic Life Care - Demographic Form must be completed prior to start
