- BLS - Basic Life Support
- Nursys - Prefer Compact Nursing License
- State or Compact License
- NIH Certification - if required within X days
- Additional Details
- COVID-19 vaccine required, provide vaccination card with submission. Declinations not accepted.
- Influenza vaccine required. Review for religious or medical exemption only.
- Physical capacity statement required stating the candidate is capable of performing the job.
- Standard 10-panel drug screen required.
- Criminal background check including county, national, FACIS level 3, OFAC, OIG, SAM, SSN trace, and education verification every 3 years.
- E-Verify and supporting documentation must be completed prior to start.
- I-9 with supporting documentation completed prior to start.
- Completion of Workplace Safety Module prior to start and annually thereafter.
- Norton KY TB Risk Assessment Form completed 90 days prior to start and updated annually.
